This is a ready-to-use recall termination package, the document a recall coordinator assembles to request that FDA confirm a recall complete. It pulls together the chronology, the communications and effectiveness-check results, the closed reconciliation, the disposition of returned or corrected product, and the status of the root cause and CAPA, into the single record FDA and your own file both rely on. Replace every <<FILL: ...>> placeholder with your own specifics. A worked filled specimen follows. This document is educational and general; confirm the current termination request process and format with your regulatory affairs function and against the current published regulation before you rely on any of it.
Use this document with the recall and field action plan, which owns the underlying recall procedure, and the recall reconciliation ledger, which supplies the closed reconciliation this package summarizes.

1. Purpose and basis for the request
State plainly that this package requests confirmation that the recall identified above is terminated, on the basis that reasonable efforts have been made to remove or correct the violative product per the recall strategy, and that it is reasonable to conclude the product has been removed or corrected commensurate with the degree of hazard it presented. This request is submitted to the FDA district office that received the original recall notification, together with the firm’s most current recall status report, per the request process described in 21 CFR 7.55.
2. Product and recall identification

Where non-responders remain unresolved, state that explicitly here rather than in a footnote; a termination request that omits an open non-responder is a different document from one that discloses it and explains why the overall action is still reasonable to close.
5. Reconciliation summary
Pull this section from the completed recall reconciliation ledger; do not restate numbers here that do not match that ledger.

| Field | Entry |
|---|---|
| Original health hazard evaluation reference | <<FILL>> |
| Original class assigned | <<FILL>> |
| Did execution or investigation change the hazard assessment? | Yes / No |
| If yes, what changed and the revised assessment | <<FILL>> |
| Confirmation the classification remains appropriate at closure | <<FILL>> |
8. Root cause and CAPA status
| Field | Entry |
|---|---|
| Root cause determined | <<FILL>> |
| Linked investigation/deviation reference | <<FILL>> |
| CAPA reference(s) | <<FILL>> |
| CAPA implementation status | <<FILL: implemented / in progress with target date>> |
| CAPA effectiveness verification status | <<FILL: verified effective / verification scheduled for [date]>> |
| Whether other lots or products sharing the cause were assessed | <<FILL>> |
A recall can be requested for termination with CAPA effectiveness verification still in progress, since effectiveness verification is often necessarily a longer-horizon measurement than the recall action itself; state the verification’s scheduled completion rather than leaving CAPA status silent.
9. Termination request statement
<<FILL: COMPANY NAME>>requests termination of recall<<FILL: recall reference>>for<<FILL: product, lot(s)>>, initiated<<FILL: date>>. Reasonable efforts have been made to remove or correct the affected product in accordance with the recall strategy on file. Effectiveness checks are complete at the level assigned, with the disposition of any remaining non-responders described in section 4. Reconciliation, summarized in section 5, is closed within the tolerance stated, with any residual explained and risk-assessed. Returned and affected product has been dispositioned as described in section 6. This package, together with the firm’s most current recall status report, is submitted per 21 CFR 7.55 requesting the appropriate FDA district office’s written confirmation of termination.
10. Acceptance criteria for submitting this request
- Effectiveness checks are complete at the assigned level, and any remaining non-responder is disclosed with its escalation status, not omitted.
- Reconciliation is closed within the stated tolerance, with every residual explained and risk-assessed.
- All returned or affected product has a documented, QA-approved disposition; no recalled product has been returned to saleable stock without one.
- Root cause is determined (or the reason it is not yet determined is stated) and CAPA status, implemented or in progress with a target date, is disclosed.
- The health hazard assessment has been reconfirmed, not merely carried forward unexamined from the original evaluation.
- The package is approved by QA before submission.

The following shows the package completed for the Class II lyophilized biologic recall used elsewhere in this topic’s materials. The company, product, people, and numbers are illustrative; replace them with your own.
Header: Document number RTP-2026-004, recall reference REC-2026-004, Class II, prepared by M. Duarte (Recall Coordinator), 29 August 2026.
Section 2, identification: Fictional Bio lyophilized biologic, 100 mg/vial. Licensed biological product under BLA <<FILL>>. Lot LB-4417. Depth: consumer/user (hospital pharmacy level). Recall initiated 14 July 2026. FDA recall coordinator notified 6 July 2026 (at complaint intake, ahead of the formal recall decision). Reason: elevated high molecular weight species and appearance failure on reconstitution, traced to incomplete secondary drying following a lyophilizer shelf temperature excursion.
Section 3, chronology (selected rows): 6 Jul 2026, complaint received and awareness date. 6 Jul 2026, health hazard evaluation HHE-2026-004 completed, Class II recommended. 7 Jul 2026, recall referred to committee. 17 Jul 2026, BPDR filed (BPDR-2026-0021), within the 45-day clock from 6 Jul. 14 Jul 2026, recall communications issued to 2 direct hospital consignees. 24 Jul 2026, effectiveness checks completed, both consignees verified. 24 Jul 2026, reconciliation closed at 100 percent (230 units quarantined, none distributed to a third party). 24 Jul 2026, root cause determined (lyophilizer probe drift, incomplete secondary drying). 12 Aug 2026, CAPA-2026-0203 implemented (calibration interval shortened, impact assessment procedure revised). 29 Aug 2026, date of this termination request. CAPA effectiveness verification scheduled for completion 12 Aug 2027 (12-month complaint rate monitoring per the CAPA plan).
Section 4, communications and effectiveness checks: 2 direct consignees notified by phone and written letter. Effectiveness-check level: A (100 percent of consignees, appropriate to a distributed sterile parenteral biologic). Both consignees verified: quarantine confirmed, no doses administered from the affected units at either site beyond the units already administered before the recall notice (see section 5). No non-responders.
Section 5, reconciliation summary:
| Lot | Manufactured | Distributed | Returned | Destroyed in place | Used/dispensed | Unaccounted | Percent accounted |
|---|---|---|---|---|---|---|---|
| LB-4417 | 9,600 | 9,180 | 8,950 | n/a | 230 | 0 | 100% |
Reconciliation ledger reference: FRM-QA-041, completed 24 July 2026. The 230 “used/dispensed” units reflect doses administered to subjects before the 6 July awareness date and the subsequent recall notice; each is confirmed against site dispensing records rather than estimated. No unresolved residual.
Section 6, disposition: 8,950 returned units and 230 company-held quarantined units rejected under disposition record DISP-2026-0141 and destroyed under certificates DC-2026-0501 through DC-2026-0504. No recalled product was returned to saleable stock. No product remains pending disposition.
Section 7, health hazard restatement: Original HHE-2026-004 concluded temporary or medically reversible consequences were possible and serious harm was remote, supporting Class II. Testing confirmed sterility, endotoxin, and particulate were not implicated; the confirmed defect was aggregation-driven purity and appearance failure, not contamination. The Class II classification remains appropriate at closure; nothing learned during execution indicated a more severe hazard.
Section 8, root cause and CAPA status: Root cause: lyophilizer shelf temperature probe TT-2214 drift causing incomplete secondary drying (INV-2026-0311). CAPA-2026-0203: corrective actions (shortened calibration interval, revised impact-assessment procedure) implemented 12 Aug 2026. Effectiveness verification: scheduled, 12-month complaint rate monitoring against the pre-excursion baseline, target completion 12 Aug 2027. A retrospective review of 24 months of prior lyophilization excursion deviations closed as no-impact was completed with no further affected lots identified.
Section 9, termination request statement: Completed using the template language in section 9 above, with the specific dates, references, and figures from this specimen substituted in.
The point of this specimen is that CAPA effectiveness verification is still twelve months from completion at the time termination is requested, and the package says so plainly rather than implying the loop is fully closed. FDA confirms recall termination on the removal and correction of the product from the field, not on the completion of every downstream effectiveness measurement; disclosing the open verification with its target date is what keeps the request accurate.
Common inspection findings this document prevents
- A termination request submitted with no reconciliation reference, or with numbers that do not match the underlying ledger.
- Non-responding consignees from the effectiveness check omitted from the termination package rather than disclosed with their resolution status.
- Returned product status left ambiguous, with no explicit confirmation that none was returned to saleable stock without a documented disposition.
- The health hazard assessment carried forward unexamined, with no statement that it was reconsidered against what the investigation actually found.
- CAPA status omitted entirely, or implied as complete when effectiveness verification is still pending, without disclosing the target completion date.
- A termination request filed before reconciliation actually closes, treating the request as a formality rather than as the evidence-based conclusion of the action.
How to adapt this document
- Set your document number, recall reference, and preparer in the header.
- Pull section 5’s figures directly from your completed reconciliation ledger; never re-key numbers independently into this document.
- Where any effectiveness-check non-responder or reconciliation residual remains open at the time you would otherwise submit this request, either resolve it first or disclose it explicitly in sections 4 and 5 with its escalation status, rather than delaying the whole package indefinitely or submitting it silently incomplete.
- Confirm the current termination request process, format, and any market-specific equivalent (this template is written for the US Part 7 process) with regulatory affairs before submission.
- Confirm every regulation in section 12 against the current published version before issue.
