This is a ready-to-use record for documenting the quality unit’s real reporting line and confirming, in writing, that it is structurally independent of the functions it oversees. Most companies can describe their quality system’s procedures in detail but have never written down, in one place, who the quality head actually reports to and why that line does not compromise their authority. This record fills that gap. Replace every <<FILL: ...>> placeholder with your own specifics and route it through your normal document control, review, and approval. A worked filled specimen follows. Verify each cited regulation against the current source before you rely on it.

1. Purpose of this record
This record documents the actual reporting line of every role with GxP decision authority at the site or organization named above, confirms that the quality unit’s authority to approve or reject is not subordinate to production, commercial, or schedule pressure, and states how quality decision-making is covered when a critical role is vacant. It is the record an investigator is implicitly testing when they ask “who does the Head of Quality report to,” and it exists so that question has a documented, current answer rather than a reconstructed one.
2. Current reporting line
| Role | Name | Reports to (title) | Peer functions at same level |
|---|---|---|---|
<<FILL: e.g. Head of Quality Assurance>> | <<FILL>> | <<FILL>> | <<FILL: e.g. Head of Manufacturing, Head of Supply Chain>> |
<<FILL: e.g. QC Director>> | <<FILL>> | <<FILL>> | <<FILL>> |
<<FILL: e.g. Validation Lead>> | <<FILL>> | <<FILL>> | <<FILL>> |
<<FILL: e.g. Qualified Person (EU)>> | <<FILL>> | <<FILL>> | <<FILL>> |
3. Independence assessment
| Question | Answer | Evidence / basis |
|---|---|---|
| Does the quality unit head report to someone whose performance is measured primarily on production output, shipment volume, or schedule adherence? | Yes / No | <<FILL>> |
| Can any role outside the quality unit direct a batch disposition, deviation closure, or validation approval without quality unit agreement? | Yes / No | <<FILL>> |
| Does the quality unit control its own budget and staffing decisions, or are they subject to approval by the function it oversees? | <<FILL>> | <<FILL>> |
| Is there a documented escalation path to senior management for an unresolved quality-versus-schedule conflict? | Yes / No | <<FILL: reference the SOP or policy>> |
| Has the reporting line changed since the last review of this record? | Yes / No | <<FILL: describe change, or "none">> |
Any “Yes” in the first two rows, or “No” in the fourth, is an independence gap. Document it as a finding under <<FILL: SOP-ID for deviations or CAPA>> and assign an owner and due date; do not leave it recorded here without a corrective action reference.
4. Vacancy and backup coverage
| Critical role | Current incumbent | Documented backup | Coverage if both are unavailable |
|---|---|---|---|
<<FILL>> | <<FILL>> | <<FILL>> | <<FILL: e.g. named external consultant, or corporate quality escalation>> |
<<FILL>> | <<FILL>> | <<FILL>> | <<FILL>> |
5. Escalation path to senior management
Describe, in plain steps, how an unresolved conflict between a quality decision and a production, commercial, or schedule pressure reaches senior management, and within what timeframe.
<<FILL: e.g. Quality unit head raises the conflict directly to the Site General Manager within 1 business day.>><<FILL: e.g. If unresolved within 2 business days, escalate to the corporate Chief Quality Officer.>><<FILL: e.g. Any override of a quality decision by a non-quality role is itself logged as a quality event.>>
6. Field definitions
| Field | Definition |
|---|---|
| Reports to (title) | The role, not the individual, that the position is organizationally subordinate to for performance management and resourcing decisions. |
| Peer functions | Other functions at the same organizational level, used to confirm the quality unit sits alongside, not beneath, the functions it oversees. |
| Independence gap | Any condition where a non-quality role can direct or override a quality decision without documented, defined recourse. |
| Backup coverage | A named person or defined escalation path able to make quality decisions within the same authority scope when the primary incumbent is unavailable. |

Filled specimen
Illustrative organization, values invented. Commercial-scale oral solid dosage site, roughly 300 staff.
| Role | Name | Reports to | Peer functions |
|---|---|---|---|
| Head of Quality Assurance | A. Rios | Site General Manager | Head of Manufacturing, Head of Supply Chain, Head of Engineering |
| QC Director | S. Okoye | Head of Quality Assurance | Head of Validation |
| Validation Lead | P. Singh | Head of Quality Assurance | QC Director |
Independence assessment: quality unit head does not report through Manufacturing (No); no non-quality role can direct disposition without quality agreement (No); quality unit controls its own hiring and budget within corporate limits (Yes, within approved headcount plan); documented escalation path exists per SOP-QA-002 (Yes); reporting line unchanged since last review (Yes).
Vacancy coverage: Head of Quality Assurance backup is the corporate VP of Quality, engaged within 24 hours of any extended absence; QC Director backup is the Senior QC Supervisor, authorized for routine decisions only, with any disposition-level decision held for the Head of Quality Assurance’s return or the corporate backup.
This specimen shows a clean result: no independence gap, an escalation path that exists on paper and has actually been used once (a schedule dispute over a cleaning validation delay, resolved at the Site General Manager level within the stated timeframe, logged as quality event QE-2026-0087). A record with no evidence the escalation path has ever been exercised is not itself a finding, but reviewers should be able to explain whether that means no conflict has occurred or that the path is untested.
Common inspection findings this record prevents
- No documented answer to “who does the Head of Quality report to,” forcing staff to describe the reporting line inconsistently under questioning.
- A reporting line that has drifted from what the quality manual describes, discovered when an investigator compares the two.
- An independence gap known informally within the organization but never formally assessed, escalated, or tracked to resolution.
- A critical quality role with no documented backup, so an extended absence creates an undocumented gap in decision authority rather than a planned, covered transition.
How to adapt this record
- Set the scope, site, and document number in the header, and list every role with GxP decision authority in section 2, not only the most senior one.
- Answer section 3 honestly against the real organization chart, not the aspirational one; a “Yes” that should be “No” here is worse than an open finding, because it hides the finding from your own quality system.
- Name real backups in section 4 before this record is approved, not as a placeholder to fill in later.
- Review this record on a fixed interval and on any organizational change, and link it to the GxP quality roles and sign-off authority matrix and the signature authority and delegation log so structure, decision rights, and individual authorizations stay consistent with each other.
