2. Field definitions
- Contact plate: an agar plate pressed to a gown surface after donning to recover viable organisms; incubated and counted in CFU.
- Gown location: a defined body location sampled at each assessment; the fingertip prints are the most sensitive and the ones inspectors examine first.
- Surface limit: the grade-based action limit for viable recovery; grade A limits are effectively zero, grade B carries a small numeric action limit (
<<FILL: your validated limit, e.g. <=5 CFU>>). - Consecutive successful assessments: three qualification runs, each meeting all location limits, with no failing run in between.
3. Gowning assessment results
Record contact-plate CFU at each location for three consecutive assessments. Every location must meet the limit in every run.

Note: an operator working directly in grade A (gloved hands inside an isolator or under laminar flow) is held to the grade A target, which is effectively zero.

Access to the aseptic area is gated on both current gowning qualification and current media-fill participation. An operator without both is not qualified to work the line.
5. Requalification triggers
Requalify on any of the following, not only on the periodic due date:
- The periodic interval (commonly annual) has elapsed.
- An EM excursion is attributable to the person.
- A documented gowning or aseptic-technique deficiency.
- Return from extended leave beyond

Common inspection findings this form prevents
- An operator working aseptically with lapsed gowning qualification or lapsed media-fill currency.
- Qualification granted on fewer than three consecutive successful runs.
- Fingertip prints not sampled, or sampled but not held to the tightest limit.
- No requalification after an EM excursion attributable to the person or after extended leave.
- Grade A operators held only to the grade B limit rather than the effectively-zero grade A target.
