Checklist: Combination Product Constituent Release Reconciliation

A plug-and-play release checklist for a combination product where a drug or biologic constituent and a device constituent converge at packaging: batch record completeness, specification conformance, stability and dating support, and lot traceability, reconciled across both constituent histories before the kit ships, with a filled specimen.

Document type: Checklist

Read and copy the template below into your own quality system. It is a generic starting point for your own internal use, provided as is, with no warranty; see the Terms and License. Adopting it does not by itself create compliance.

This is a ready-to-use release reconciliation checklist for a combination product whose constituents carry separate manufacturing histories that converge at packaging, a co-packaged kit, a single-entity product with a separately-lotted device component, or any configuration where drug and device evidence must be reconciled before one release decision covers both. It is the working artifact behind the release rule stated in the combination product quality plan, section 6. Replace every <<FILL: ...>> placeholder with your own specifics. Mark each item Pass, Fail, or N/A and route the completed checklist through your normal quality release process. A worked filled specimen follows. Verify each cited regulation against the current source before you rely on it.

Before you start

Confirm which constituents this release decision covers and where their manufacturing histories are recorded, before working the checklist item by item.

Reconciliation checklist

#ItemPass / Fail / N/AComment
1Drug or biologic constituent batch record is complete, reviewed, and approved under the drug cGMP base (Part 211, or Parts 600-680 for a licensed biologic).
2Device constituent history record or device master record evidence is complete, reviewed, and approved under the added or base device provisions (21 CFR 4.4, QMSR / ISO 13485).
3Drug or biologic constituent meets its release specification (assay, sterility, endotoxin, appearance, potency, as applicable).
4Device constituent meets its release specification (dimensional, functional / delivery performance, package integrity, as applicable).
5Stability data supports the labeled expiry of the finished kit or product, driven by the shorter-lived constituent.
6For sterile products, container closure integrity is demonstrated over the labeled shelf life.
7Drug or biologic constituent lot number is captured on the kit or finished-product record.
8Device constituent lot or serial number is captured on the kit or finished-product record.
9Quantities of each constituent reconciled against quantities issued to the packaging or assembly step, with any discrepancy investigated and closed.
10Any open deviation, CAPA, or complaint trend against either constituent has been checked and does not implicate this batch or lot.
11Labeling correctly identifies both constituents where required and matches the approved labeling for the combination product.
12If this is a co-packaged kit, the assembly or kitting step itself has a completed, reviewed batch record reconciling the constituents brought together.

Filled specimen

The following shows the checklist completed for a co-packaged kit: a vial of lyophilized biologic, a prefilled diluent syringe, and a reconstitution needle, assembled into one carton and released as one product. The company, product, and numbers are illustrative.

Common inspection findings this checklist prevents

  • The kit is released on the drug constituent’s record alone, with the device components treated as bought-in stock and never reconciled.
  • A quantity variance at kitting is left uninvestigated because no single record forces the reconciliation.
  • The labeled expiry is set from the drug constituent’s stability data without confirming the device component’s shelf life actually supports it.
  • Lot traceability is incomplete, so a later investigation cannot reconstruct which device lot was paired with which drug lot.

How to adapt this checklist

  1. Adjust items 3, 4, and 6 to the actual specifications and test methods for your specific constituents; not every combination product has a sterility or CCIT requirement.
  2. If your configuration is single-entity rather than co-packaged, drop item 12 and confirm which items still apply to your assembly step.
  3. Set your own release-decision rule in the release decision section to match your quality system’s language.
  4. Point the References section at your own internal batch record and specification procedures alongside the regulatory citations.
